Hope all is well.

My wife gave birth on the 26th of October 2024 and I requested the UI19, form to claim Paternity benefits as it was 10 days which is also equal to half of my monthly salary.

Today is the 28th of November month later and I still have not received my UI19 form and my manager or operation’s is doing nothing to assist or follow up on this matter from HR

Could you please kindly assist me or guide me in right direction. I’m already in financial predicament.

I was told to send birth certificate first then only could HR assist me so I sent birth certificate and still haven’t received anything a month later.

Kindly please assist me my previous company doesn’t want to provide me with UI-19 forms they giving me different excuses that i don’t understand.

Please advise what to do.

The company I worked for retrenched all of its employees in December 2024. To date we were not given our UI 19 forms to claim for the UIF, and we were not being given our payslips. When I check my eFiling, it was shows that my UIF was last paid in April 2024, I had reported to the management but they couldn’t get back to me as their company that was assigned to do our payrolls is based in Capetown and we were Located in Gauteng. I managed to get hold of the company that was responsible for our payroll to raise my inquiry but they’ve been ignoring me.

My previous employer that I left 2023 is refusing to issue a UI 19 and salary schedule. They are coming with excuses, that they changed payroll system.

Have provided them with the necessary documents employee number and my ID number.
