Hope all is well.

My wife gave birth on the 26th of October 2024 and I requested the UI19, form to claim Paternity benefits as it was 10 days which is also equal to half of my monthly salary.

Today is the 28th of November month later and I still have not received my UI19 form and my manager or operation’s is doing nothing to assist or follow up on this matter from HR

Could you please kindly assist me or guide me in right direction. I’m already in financial predicament.

I was told to send birth certificate first then only could HR assist me so I sent birth certificate and still haven’t received anything a month later.
